Additional Parties - Detailed Policy

1. Purpose
This policy outlines the fees and procedures for including additional parties in DNA testing services provided by Lineage DNA Testing.
2. Scope
This policy applies to all DNA testing services offered by Lineage DNA Testing where more than two parties are involved in the testing process.
3. Base Fee Coverage
3.1. The standard base testing fee covers the inclusion of up to two (2) parties in the DNA testing process.
3.2. Typically, these two parties consist of:
a) One alleged father, and
b) One child
3.3. Any variation in the composition of the two parties covered by the base fee must be explicitly approved by Lineage DNA Testing prior to testing.
4. Additional Party Fee
4.1. For each party beyond the two covered by the base fee, an additional fee of $100.00 (one hundred dollars) will be applied.
4.2. Additional parties may include, but are not limited to:
a) Additional children
b) Mothers
c) Potential fathers
d) Other relatives relevant to the DNA testing process
4.3. The additional party fee applies uniformly regardless of the relationship of the additional party to the primary testing subjects.
5. Group Discounts
5.1. Lineage DNA Testing may offer discounts for cases involving more than three additional parties.
5.2. Group discounts are not automatically applied and must be requested by the client.
5.3. To inquire about group discounts, clients must contact the Lineage DNA Testing office directly for a customized quote.
5.4. Group discount rates are determined on a case-by-case basis and are subject to the discretion of Lineage DNA Testing management.
6. Documentation Requirements
6.1. All parties participating in the DNA testing process, including those covered by the base fee and additional parties, must provide:
a) Valid, government-issued photo identification
b) Completed and signed consent forms as provided by Lineage DNA Testing
6.2. Failure to provide the required documentation may result in delays or cancellation of the testing process.
6.3. Lineage DNA Testing reserves the right to refuse testing for any party who does not comply with the documentation requirements.
7. Payment
7.1. All fees, including the base fee and any additional party fees, must be paid in full prior to the commencement of DNA testing services.
7.2. Payment must be made through approved payment methods as designated by Lineage DNA Testing.
8. Refunds and Cancellations
8.1. In the event that an additional party is unable to participate in the testing process after fees have been paid, Lineage DNA Testing will assess the situation on a case-by-case basis to determine if a refund of the additional party fee is warranted.
8.2. Any refunds issued are at the sole discretion of Lineage DNA Testing management.
9. Policy Modifications
9.1. Lineage DNA Testing reserves the right to modify this Additional Parties Policy at any time without prior notice.
9.2. Any changes to this policy will be communicated to clients through appropriate channels and will be effective immediately upon publication.
This policy is effective as of July 1, 2024 and supersedes any previous policies regarding additional parties in DNA testing issued by Lineage DNA Testing.
