1. Purpose
 

This policy outlines the terms and conditions for cancellation and rescheduling of appointments with Lineage DNA Testing.

2. Scope
 

This policy applies to all clients who have scheduled DNA testing services with Lineage DNA Testing.

3. Standard Cancellation
 

3.1. Clients may cancel their scheduled appointment without incurring any penalties if the cancellation is made at least forty-eight (48) hours prior to the scheduled appointment time.
 

3.2. Cancellation requests must be submitted in writing via email or through the official Lineage DNA Testing website.
 

3.3. The date and time of the cancellation request receipt will be used to determine compliance with the 48-hour requirement.

4. Late Cancellations
 

4.1. Cancellations made less than forty-eight (48) hours but more than twenty-four (24) hours before the scheduled appointment will be considered late cancellations.
 

4.2. Late cancellations will incur a cancellation fee of $100.00 (one hundred dollars).
 

4.3. This fee must be paid before scheduling any future appointments with Lineage DNA Testing.

5. No-Show Policy
 

5.1. A "no-show" occurs when a client fails to appear for a scheduled appointment without providing any prior cancellation notice.
 

5.2. In the event of a no-show, the client will be charged the full service fee for the scheduled appointment.
 

5.3. No refunds will be issued for no-show appointments under any circumstances.

6. Rescheduling
 

6.1. Clients may reschedule their appointment once without incurring any penalties, provided that:
 

a) The rescheduling request is made at least twenty-four (24) hours prior to the original appointment time.
b) The rescheduling is for the same type of service originally booked.
 

6.2. Subsequent rescheduling requests or requests made less than 24 hours before the appointment may be subject to fees or penalties at the discretion of Lineage DNA Testing.

7. Extenuating Circumstances
 

7.1. Lineage DNA Testing may, at its sole discretion, waive cancellation fees or no-show charges in cases of documented emergencies or extenuating circumstances.
 

7.2. Clients must provide appropriate documentation to support any claim of extenuating circumstances within 72 hours of the missed appointment.

8. Cancellation and Rescheduling Procedure
 

8.1. All cancellation and rescheduling requests must be submitted in writing via email or through the official Lineage DNA Testing website.
 

8.2. Requests must include the client's full name, appointment date and time, and reason for cancellation or rescheduling.
 

8.3. Verbal cancellations or rescheduling requests will not be accepted or honored.

9. Payment of Fees
 

9.1. Any fees incurred due to late cancellations or no-shows must be paid in full before scheduling any future appointments with Lineage DNA Testing.
 

9.2. Payment must be made through approved payment methods as designated by Lineage DNA Testing.

10. Dispute Resolution
 

10.1. In the event of a dispute regarding cancellation fees or no-show charges, clients are encouraged to contact Lineage DNA Testing customer service for resolution.
 

10.2. If a resolution cannot be reached, the dispute will be subject to the jurisdiction of [insert relevant local court or arbitration body].

11. Policy Modifications
 

11.1. Lineage DNA Testing reserves the right to modify this Cancellation Policy at any time without prior notice.
 

11.2. Any changes to this policy will be communicated to clients through appropriate channels and will be effective immediately upon publication.

This policy is effective as of July 1, 2024 and supersedes any previous cancellation policies issued by Lineage DNA Testing.
