1. Purpose
 

This policy outlines the terms and conditions for requesting and booking same-day appointments with Lineage DNA Testing.

2. Scope
 

This policy applies to all clients seeking same-day appointments for DNA testing services provided by Lineage DNA Testing.

3. Urgency Fee
 

3.1. A same-day appointment may be arranged for an additional urgency fee of $50.00 (fifty dollars).
 

3.2. This fee is subject to the availability of staff and resources, as determined by Lineage DNA Testing.
 

3.3. The urgency fee is in addition to any standard fees associated with the requested DNA testing services.

4. Booking Window
 

4.1. Same-day appointment requests must be made at least four (4) hours prior to the desired appointment time.
 

4.2. Requests for same-day appointments received after 2:00 PM local time may be subject to an additional fee, to be determined at the discretion of Lineage DNA Testing.
 

4.3. Lineage DNA Testing reserves the right to refuse same-day appointment requests made outside of the specified booking window.

5. Availability
 

5.1. Same-day appointments are not guaranteed and are contingent upon the availability of Lineage DNA Testing staff and resources.
 

5.2. Lineage DNA Testing will make reasonable efforts to accommodate same-day appointment requests but reserves the right to decline such requests based on operational constraints.
 

5.3. In the event that a same-day appointment cannot be accommodated, Lineage DNA Testing will offer the next available appointment slot.

6. Payment
 

6.1. The urgency fee of $50.00 must be paid in full at the time of booking the same-day appointment.
 

6.2. Payment must be made through approved payment methods as designated by Lineage DNA Testing.
 

6.3. Failure to pay the urgency fee at the time of booking may result in the cancellation of the same-day appointment request.

7. Cancellation and Refunds
 

7.1. The urgency fee is non-refundable once the same-day appointment has been confirmed by Lineage DNA Testing.
 

7.2. In the event that Lineage DNA Testing is unable to fulfill a confirmed same-day appointment, a full refund of the urgency fee will be issued to the client.

8. Modifications to the Policy
 

8.1. Lineage DNA Testing reserves the right to modify this Same Day Appointment Policy at any time without prior notice.
 

8.2. Any changes to this policy will be communicated to clients through appropriate channels and will be effective immediately upon publication.

9. Compliance
 

9.1. By requesting and booking a same-day appointment, clients agree to comply with all terms and conditions outlined in this policy.
 

9.2. Lineage DNA Testing reserves the right to refuse service to any client who fails to comply with this policy.

 

This policy is effective as of July 1, 2024 and supersedes any previous same-day appointment policies issued by Lineage DNA Testing.
