• Included mileage fee: No fee for travel, 50 miles and under (RT - round trip), from the office/provider location

• Standard mileage fee: $0.67/mile for travel between 51 - 75 miles (RT) from office/provider location PLUS TOLLS
• Extended travel fee: $1.00 per mile for distances 76 miles and over PLUS TOLLS
• Mileage calculated using Google Maps for round trips
• Fees may be waived for bulk testing contracts or special agreements
• Policy subject to annual review and potential amendments

1. Purpose
 

This policy outlines the mileage reimbursement and associated fees for travel required by Lineage DNA Testing employees and contractors in the course of their duties.

2. Scope
 

This policy applies to all employees and contractors of Lineage DNA Testing who are required to travel for business purposes.

3. Standard Mileage Rate
 

3.1. A mileage fee of $0.67 per mile, in accordance with the current Internal Revenue Service (IRS) standard mileage rate, will be applied for all business-related travel.

3.2. This fee will be charged for travel that extends beyond a 25-mile radius from either:
 

a) The main office of Lineage DNA Testing, or
b) The service provider's primary location, whichever is applicable.

 

4. Extended Travel Provisions
 

4.1. For travel distances exceeding 50 miles, Lineage DNA Testing reserves the right to charge an additional fee for travel time.

4.2. This additional fee will be calculated at an hourly rate of $30.00 per hour.
 

4.3. The travel time will be determined based on the estimated duration of the journey as calculated by Google Maps or a similar reputable mapping service.

5. Mileage Calculation
 

5.1. All mileage calculations will be performed using Google Maps or an equivalent mapping service approved by Lineage DNA Testing management.
 

5.2. The mileage will be calculated based on the round trip distance:
 

a) From the starting point to the appointment location, and
b) From the appointment location back to the starting point.
 

5.3. The starting point shall be defined as either the main office of Lineage DNA Testing or the service provider's primary location, whichever is applicable to the specific travel instance.

6. Waivers and Exceptions
 

6.1. Lineage DNA Testing reserves the right to waive mileage fees under certain circumstances, including but not limited to:
 

a) Bulk testing contracts
b) Special agreements negotiated with clients
 

6.2. Any waiver of mileage fees must be explicitly agreed upon in writing between Lineage DNA Testing and the client prior to the commencement of services.

7. Policy Review and Amendments
 

7.1. This policy will be reviewed annually by Lineage DNA Testing management.
 

7.2. Lineage DNA Testing reserves the right to amend this policy at any time. Any changes will be communicated to all employees and contractors in a timely manner.

8. Compliance

8.1. All employees and contractors are expected to comply with this policy.
 

8.2. Any deviations from this policy must be approved in writing by authorized Lineage DNA Testing management.

This policy is effective as of July 1, 2024 and supersedes any previous mileage policies issued by Lineage DNA Testing.
