1. Purpose
This policy outlines the terms and conditions under which Lineage DNA Testing will issue refunds for its DNA testing services.
2. Scope
This policy applies to all clients who have scheduled and paid for DNA testing services with Lineage DNA Testing.
3. Refund Eligibility
3.1. Clients are eligible for a refund if a cancellation request is received by Lineage DNA Testing at least forty-eight (48) hours prior to the scheduled appointment time.
3.2. Cancellation requests must be submitted in writing via email or through the official Lineage DNA Testing website.
3.3. The date and time of the cancellation request receipt will be used to determine eligibility for a refund.
4. Processing Time
4.1. Approved refunds will be processed within ten (10) business days from the date of the cancellation request.
4.2. The refund will be issued using the original method of payment unless otherwise specified and agreed upon by both parties.
4.3. Lineage DNA Testing is not responsible for any delays in refund processing due to banking or credit card company procedures.
5. Non-Refundable Fees
5.1. The following fees are non-refundable under any circumstances:
a) Same-day appointment fees
b) Mileage fees
c) Lab processing fees
5.2. These non-refundable fees will be deducted from the total amount paid when calculating any refund due.
6. Partial Refunds
6.1. If DNA testing has commenced but not been completed at the time of cancellation, a partial refund may be issued.
6.2. The partial refund will exclude costs for services already rendered, including but not limited to:
a) Sample collection fees
b) Partial lab processing fees
c) Administrative costs
6.3. The amount of the partial refund will be determined by Lineage DNA Testing management on a case-by-case basis.
7. Refund Request Procedure
7.1. To request a refund, clients must:
a) Submit a written request to Lineage DNA Testing via email or through the official website
b) Include their full name, appointment date, and reason for cancellation
c) Provide any relevant supporting documentation
7.2. Lineage DNA Testing will review the refund request and communicate the decision to the client within five (5) business days of receipt.
8. Exceptions
8.1. Lineage DNA Testing reserves the right to make exceptions to this policy in extenuating circumstances.
8.2. Any exceptions will be evaluated on a case-by-case basis and are at the sole discretion of Lineage DNA Testing management.
9. Dispute Resolution
9.1. In the event of a dispute regarding a refund, clients are encouraged to contact Lineage DNA Testing customer service for resolution.
9.2. If a resolution cannot be reached, the dispute will be subject to the jurisdiction of Gloucester County Courthouse, Civil Division, Woodbury, New Jersey 08096.
10. Policy Modifications
10.1. Lineage DNA Testing reserves the right to modify this Refund Policy at any time without prior notice.
10.2. Any changes to this policy will be communicated to clients through appropriate channels and will be effective immediately upon publication.
This policy is effective as of July 1, 2024 and supersedes any previous refund policies issued by Lineage DNA Testing.
